The Protection of Personal Information Act of 2013 (POPIA) allows for any Data Subject to find out from any Responsible Party, what Personal Information relating to the Data Subject is held by the Responsible Party, and also the third parties or categories of third parties who have had access to the Personal Information.  This form enables you to request that information from Lightstone (Pty) Ltd in accordance with Section 23 of POPIA.  

Please note that only the Data Subject may submit this request; no other person may submit on their behalf. By submitting this form you confirm that you are the Data Subject.  We are required to verify your identity, so please have a copy of your ID document (or Company Registration documents if enquiring as a Juristic Entity) to hand.  You may be required to make a payment before we process your request; this is in part to cover costs of the service to verify your identity in order to be sure we are sending your PI to you, and to cover our costs of running this service.
